Once completed the form can be uploaded by logging into your Resident Life Profile. We will strive to meet all medical accommodations; however, we cannot guarantee other preferences.
Please be aware that if circumstances change and the situation necessitates, the Department of Resident Life reserves the right to modify or cancel students’ housing agreements. Students will be notified immediately if this is the case.
